GPU not showing up, what causes this?

I have a Linux mining rig working with 11 GPUs, when I connect the 12th GPU it sometimes fails to post and other times boots but the GPU is not listed via lspci.

I have done the usual stuff like disabling data, enabling 4g, updating to latest bios, forcing PCIe to gen1.

This background to ask, what is causing the GPU not to load, I am using plx multiplexers to split lanes on a Ga-h110-d3a motherboard so my expectation is that I won't be using all the PCIe lanes in the chipset (6) since I only have 4 things connected to the 5 chipset slots. The 16x has the 3rd multiplexer but I assume this will be fine on the dedicated cpu lanes.

I have a friend who is using only 2 multiplexers on the same motherboard and is running 12 cards, which makes me even more confused.

Did you individually test each PCIE riser?

I've heard that reinstalling OS with all GPUs plugged in can help, but ..meh
